Hepatitis B — global risk, prevalent in Africa & Asia

Transmission

Hepatitis B spreads through blood, body fluids and from mother to baby. Traveller risk comes from medical or dental procedures, tattoos, piercings, intimate contact, or accidents involving blood exposure in endemic regions.

Geography

Endemic across large parts of sub-Saharan Africa, Asia and the Pacific. UK-born children since 2017 are routinely vaccinated, but most adults travelling for long stays in high-prevalence regions need protection.

Immunity

Protection requires three doses. Standard 0–1–6 months gives the strongest response; accelerated 0–7–21 days with a 12-month booster is the choice if you’re leaving sooner. After a complete course, immunity is typically lifelong.

Symptoms to watch for

Symptoms of acute Hepatitis B.

Many people have no symptoms. If they appear, onset is 6 weeks–6 months after exposure.

Fever

Low-grade, often with chills.

Fatigue

Profound and persistent.

Nausea

With loss of appetite.

Jaundice

Yellow skin and eyes.

Dark urine

Tea-coloured.

Joint pain

Pre-jaundice phase in some cases.

Eligibility checklist

You should book Hepatitis B vaccine if…

  • You are travelling long-stay in sub-Saharan Africa, much of Asia or the Pacific.
  • You are a medical, aid or expedition worker with possible exposure risk.
  • You plan tattoos, piercings or intimate contact abroad.
  • You will undertake high-risk activities (motorbike touring, extreme sports).
  • You are any age — Hep B is licensed from birth.
Severe allergy to a previous Hep B dose or to vaccine yeast (yeast-derived recombinant) excludes. Pregnancy and breastfeeding don’t exclude where exposure risk is real.

If you also need Hep A, Twinrix covers both with the same number of injections — usually cheaper and easier. If you only need Hep B, single-antigen vaccine is fine.
After a complete 3-dose course, immunity is typically lifelong for healthy adults. No routine boosters needed.
